INFORMATION FOR PATIENTS

This is a section is to provide reference information for my patients.

Jan 10, 2022: New practice location

I have started working at a new clinic effective Jan 10, 2022. The updated location and contact information has been provided to my patients.

I am seeing only my patients while I transition into this clinic. The electronic chart transfer from my previous clinic is ongoing.

I have brought over my wait list from my previous clinic though I am not accepting new patients during this transition time. I plan to resume accepting patients once I'm settled in the clinic.

Given the recent changes in the COVID-19 pandemic, I am doing only telephone visits until further notice.
